From a surgical perspective, a natural-looking nose has gentle transitions between each aesthetic sub-unit of the nose: the bridge, the tip, the alae, and the columella. The transitions should be soft and free from extreme angles or sharp lines. With some rhinoplasty procedures, the nose has been made to look unnatural with transitions between the different subunits that look artificial. Therefore the primary aim of rhinoplasty surgery is to create gentle transitions between sub-units and to take into account the patient’s goals, facial features and nasal characteristics.
Before Dr. Solomon recommends any cosmetic surgical procedures, he first schedules a consultation at either our Toronto or Thornhill clinic. This allows him to discuss your personal goals and review your health history.

Your revision rhinoplasty surgery is performed under local or general anesthesia. Preparations are made based on the type of anesthesia required to keep you comfortable during the procedure.
Because each case is unique, it is difficult to explain the basic steps involved. However, reconstruction of the underlying nasal structure may be required using cartilage grafts from your nose, ear, or rib.
Other corrective steps might require fascia from the scalp to cover imperfections on the nasal tip or bridge while cartilage grafts may also be required for reconstructive revisions such as your nasal tip.
In some cases, implant material such as Gore-Tex or Medpore may be considered, whether because natural tissue is unavailable or you do not wish to undergo surgical procedures to remove cartilage from your ear or rib. Reconstruction and reshaping the framework of bone and cartilage and re-draping of the skin are typical requirements for revision rhinoplasty.
Following your revision rhinoplasty surgery, you will need someone to pick you up to ensure your safety. Depending on the patient and the extent of the surgery, Dr. Solomon may use traditional or dissolvable stitches/sutures or both. If traditional stitches are used, Dr. Solomon or one of his Registered Nurses will arrange an appointment at a suitable time to remove them.
Because secondary surgeries are far more personalized, Dr. Solomon will review post-procedure expectations regarding bandages, bruising, swelling, bleeding, etc. and how to relieve symptoms based on those expectations.
The potential for rejection or infection is one of the possible issues with grafts and implants. Dr. Solomon will discuss the risks based on the procedure. Harvesting a rib for a cartilage graft can be uncomfortable; patients may be unwilling to accept a chest incision, and the graft may shrink or bend with time. A cadaveric rib graft can be an alternative option for some patients who don’t want to use their own rib cartilage.
All of the specific and possible side effects will be reviewed with you both at your consultation so you can decide whether you are comfortable with the risks, as well as prior to surgery, so you understand what to expect.
Although rhinoplasty is considered a safe procedure, revision rhinoplasty is often less routine than the primary surgery. As a result, it might present unique risks. With any surgery, there are risks however, with revision rhinoplasty, there can be increased risks as the nasal structure has already experienced previous trauma. Bleeding, structural issues, scar tissue and difficulty breathing are all possible side effects from revision rhinoplasty. It’s imperative that the surgeon feels the benefits outweigh the risks for a revision rhinoplasty.
Rhinoplasty surgery does require time to heal. While most noses can look great in the early weeks and months following rhinoplasty, the nasal structure is not fully healed for one year. Therefore, a revision surgery can only be considered once the nose is fully recovered.
A revision rhinoplasty is typically carried out under general anesthesia and can take anywhere from 2 – 5 hours, depending on the complexity of the case and the surgeon’s experience and skill. Often, only highly trained and skilled surgeons will take on a revision rhinoplasty that was performed by another surgeon.
