Designed to refine facial proportion, hairline lowering is gaining attention. Here’s what to know about the procedure, whom it’s for and what to expect.

For those who feel their forehead appears elongated and pronounced, there’s a lesser-known cosmetic procedure that can quietly restore balance. Hairline lowering surgery gently brings the hairline forward, helping create more harmonious facial proportions with natural-looking results. But what exactly does hairline lowering involve, and how does it differ from a hair transplant? For readers who may be unfamiliar with the procedure, what is hairline lowering or forehead reduction?

Hairline lowering, also known as forehead reduction, is a cosmetic surgical procedure designed to reduce the height of the forehead by advancing the hairline forward. The goal is to create better facial balance and proportion. This procedure is most often sought by individuals who feel their naturally high hairline makes their forehead appear disproportionately long.

Who are typical candidates for this procedure? Is hairline lowering primarily sought by women, or are you seeing increasing interest from men as well?

Ideal candidates are individuals whose forehead height appears long relative to their facial features and who have a stable hairline. It’s important that patients are not experiencing active hair loss, as this could compromise the long-term aesthetic outcome. 

While hairline lowering is most commonly performed on women, we also see interest from transgender patients and select men. As men age, hairlines often recede, which may expose a surgical scar over time. For this reason, the procedure is generally better suited for women and transitioning patients who tend to have more stable hairlines as the male hormone testosterone can lead to male pattern hair loss.

What does the surgery involve, and what does the recovery process look like? How long before patients see natural-looking results?

The procedure is performed under general anesthesia at our Toronto surgical centre and typically takes about two hours. An incision is made along the natural hairline creating a scalp flap that is carefully dissected to allow for maximum mobility, enabling the hairline to be advanced forward—usually by one to two inches. The incision is strategically placed so it can be concealed within the hairline, allowing hair to grow through the scar over time. Patients are advised to take seven to 10 days off for rest and recovery. The improvement in hairline position is immediate, which many patients find very gratifying, while the scar continues to mature and fade over several months, typically becoming indetectable.

What is the difference between hairline lowering and a hair transplant?

Although both procedures aim to improve the appearance of the hairline, they are fundamentally different. Hairline lowering surgically repositions the existing hairline to reduce forehead height. Hair transplant surgery, on the other hand, involves redistributing hair follicles and can address such areas as the temples, crown, beard, eyebrows and scalp density. In many cases, the two procedures can work synergistically. For example, a patient with a high hairline and temple recession may undergo hairline lowering first, followed by a hair transplant to refine or fill in areas that could not be fully addressed with hairline surgery alone.

Scarring is often a concern with hairline procedures. How is the incision placed, and what techniques help keep scars discreet?

Any surgical incision results in a scar; that is unavoidable. The goal, however, is to create a scar that is thin, flat, under minimal tension, and well concealed within the hairline. Over time, hair can grow through the scar, making it increasingly difficult to detect. Patient selection plays a critical role. Individuals with thin or wispy hair may have more difficulty concealing a scar, and this is discussed thoroughly during consultation. 

Can hairline lowering be combined with other procedures?

Yes. Hairline lowering can be safely combined with other facial procedures. In our practice, it is commonly paired with a brow lift. This combination is particularly popular among transgender patients transitioning from male to female, as a lower hairline and elevated, arched brows are typically considered more feminine features and contribute to overall facial feminization. This combination may also be combined with forehead reshaping for additional feminization.

How can someone determine if they are a good candidate for hairline lowering surgery?

Good candidates typically have a naturally high but stable hairline, medium to thick hair density, and healthy scalp elasticity. During consultation, we assess scalp mobility manually to determine how much advancement can be achieved; a tight scalp may limit the degree of lowering and increase the risk of scarring. A patient’s healing history is also important, particularly if there is a tendency toward poor wound healing or hypertrophic scarring. Finally, realistic expectations are essential. When patients understand what can be safely and effectively achieved, we are best positioned to deliver optimal results.
