Rachels. I am always trying to take the accent off my nose and I am hoping that after the Skin deep rhinoplasty surgery, I will be able to get up in the morning throw my hair back and not even think about my nose shape.
Dr Solomon. Rachels main problem is that her cartilage on the top of her nose is shaped like a hump. The rhinoplasty surgery will reduce the bump and make a dramatic difference in her appearance.
Rachel So what we decided jointly is to remove the hump off my nose and remodel the cartilage at the end of my nose.
Dr Solomon. So here we have taken out some of the cartilage and some of the nasal bone. After the bridge of the nose is lowered to the appropriate level we have to deal with the 3rd component of the operation which is closing what is known as the open roof deformity. This is accomplished by fracturing the nose to allow the mobility of the nasal bones.
We have also defined the nasal tip. This has created some more nostril flaring. Finally we have closed all the incisions that have been made inside the nose. I think the surgery went quite well and after the healing process she should be quite happy.

Teenage
rhinoplasty /nose jobs are amongst the most common procedures. Age 15 is the youngest age I would generally offer rhinoplasty to a patient. Age is only one factor in determining candidacy for
cosmetic rhinoplasty. A patients maturity both physically & emotionally is most important.
There are few Canadian guidelines on what age is appropriate for surgery. Health Canada says teenage girls must be 18 before receiving breast implants. For other cosmetic surgery children under 16 must have parental consent. But before operating on teens there are important issues for

The 2 approaches to
rhinoplasty surgery one is the open approach and the other is the closed approach and in Lori’s case it’s the closed approach that we will use. I am separating the cartilage from the skin envelope here. We are removing some of the cartilage bump on Lori’s nose. This is a rasp and I am just filing things down the nasal bone run the smoothly edges to get this as smooth as possible. You will see as we go long the changes are immediate. We are going to mobilize the bone now.Before I had the surgery I was very subconscious of people seeing me from the side. And after the surgery its amazing how different I feel. I am the same person and the thing that used to bother me is not there anymore.

If a patient does not want to hear about the risks of plastic surgery and the plastic surgeon is aware of this, it is the surgeon’s obligation to tell the patient that they are not a good candidate for cosmetic surgery. This is important to protect the patient and the plastic surgeon. Cosmetic Plastic Surgery inherently has risks associated with it. Even if the surgery is carried out expertly there is the chance that there still can less than perfect results. The major risks include bleeding, infection or poor cosmetic outcome. The best chance of keeping risks to a minimum is to see a surgeon that frequently performs the surgery you are interested in. However patients must realize that even in best case scenarios there is a chance of imperfections arising from cosmetic surgery. In my practice I will do all I can to help a patient realize the best outcome I feel is possible. In some cases this may require revision or touch up surgery.
Closed Rhinoplasty
Dr. Solomon, Facial Cosmetic Surgeon, located in Toronto, performs closed rhinoplasty on a young woman in an episode of Plastic Makes Perfect. This young women presented for cosmetic rhinoplasty surgery, disliked the size and shape of her nose. Specifically, she noticed her nasal tip to be wide and bulbous and her nasal bridge to have a hump. Dr. Solomon, Cosmetic Facial Surgeon evaluates the patient, reivews the surgery and all related risks. She was noted to have a bulky nasal tip wth relatively thick nasal skin. She was appraised of the limitation of refinement of her nasal tip based on her nasal skin type. Surgery was performed under general anesthesia. Closed rhinoplasty technique was used. The cartilage of the nasal tip was delivered and trimmed. As her cartilage was vertically oriented alar rim grafts were added to the nasal tip for support to the sidewalls of her nose. The bridge was reduced. Nasal bones were mobilized with osteotomies. The nose was slightly shortened. After surgery, Dr. Solomon sees the patient for a follow up. Botox & Facial Filler
This video demostrates a woman in her fourties obtaining facial enhancement prior to her wedding. She was assessed by Dr. Solomon, Cosmetic Surgeon located in Toronto for consideration of Botox and Facial Fillers. Her evaluation revealed signs of aging in different areas of her face. She had lost volume in her cheeks, and lips. The patient had developed fee deep nasolabial folds. She was noted to have lines "gabella" lines between her eyes. Botox, Cosmetic product was used to reduce lines on her forehead and around her eyes. Teosyal, hyluronic acid, and restylane was used to rebuild her cheeks and fill in folds. Ultrapulse C02 Laser Facial resurfacing

There are so many laser products on the market it gets difficult to know which is best for you. Here we will give a brief summary of laser resurfacing technology in the hope of shedding light on the topic. C02 laser technology has been around for many years. The C02 refers to Carbon Dioxide the medium by which this laser operates. The original CO2 lasers were a surgical device. These lasers would burn or evaporate a layer of skin and allow the skin to repair itself resulting in less wrinkles and smoother more youthful appearance. There were a few problems with the original CO2 lasers as it required a significant healing time and the skin could scar or heal in with a lighter colour called hypopigmentation.
Over the past decade there has been a move to develop lasers that have less downtime, excellent results and less risk. The next generation resurfacing technology was the Erbium laser which was developed to have less healing downtime, and less risk of hypopigmentation. Overall this laser had its applications but had a much more limited ability to treat significant signs of aging and sun damaged skin. A few years ago lasers changed dramatically in their delivery system. Laser would deliver their beam in a fractionated pattern. This meant that when the laser was delivered instead of treating the entire skin surface the laser could vaporize or heat cores or holes into the skin. It was as if a laser was shot through a filter and only parts of the beam hit the skin. This allowed bridges of normal skin to not be damaged by the laser beam. The bridge of unlasered skin would allow for quicker healing and less risk of hypopigmentation. The most common devices included the Fraxel laser and the profractional laser. These worked similarly but the physical properties of each company’s laser were different. The Profractional and the Fraxel are erbium based lasers with fractionated technology. Both of these technologies are good but require multiple treatments over months to obtain noticeable results.
The newest technology on the block is the fractionated C02 laser. This technology takes the old C02 technology and applies the Fractionated delivery system. This Technology allows all the benefit of the old C02 power and lessens the risk of scarring and hypopigmentation. Most patients will notice often dramatic results from one treatment. The Downtime of this technology is about 7-10 days of red faking skin. The skin will improve from one treatment for a number of months. The two lasers of this type on the market are the Fraxel C02 laser and the UltraPulse Fractionated C02 laser. Dr Solomon has obtained the first Ultrapulse in the GTA. Both of the C02 Fractionated technologies are great and really offer the gold standard of skin resurfacing. I like the Ultrapulse as it offers flexible settings. The Ultrapulse C02 has an Active FX setting which is great for pore size, fine lines, and overall skin tightening. The Deep FX setting allows treatment of acne scars, deep wrinkles and surgical scars.. Many of the patients we treat obtain both Active FX and Deep FX treatment at the same time. We first do the Deep FX in the areas that require it and then go over the entire face with the active treatment. The results are like no other laser that I have seen. This laser is an advanced surgical tool/laser/computer and needs to be offered by a physician or surgeon. Patients must understand that this technology is different from other lasers currently on the market. It can achieve dramatic results in one to two treatments far superior to other lasers on the market. However with the upside patients have to accept 7-10 days of redness and swelling as they go through the first phase of healing.
